If you’ve been chasing end-game glory in Path of Exile 2, you’ve likely heard whispers about the Simulacrum. This Delirium-infused trial is run as a 15-wave gauntlet that tests your build’s survivability, crowd control, and consistent DPS. Your rewards scale with performance, so efficiency and discipline matter as much as raw power.
How it works
Entry and setup: To access the Simulacrum, you need Simulacrum Splinters, items earned through Delirium encounters, and a way to convert those splinters into a full entry. Once you assemble the required components, you activate the simulacrum at the map device or region portal, entering a randomized arena that changes between runs.
Wave structure: The core mechanic is a sequence of 15 waves. Each wave brings more enemies, tougher modifiers, and occasional elite or boss-like encounters. Between waves, you can manage your inventory and stash loot, but loot on the ground will disappear when the next wave begins, so quick decisions are essential.
Bosses and modifiers: Every few waves, a boss appears to test your single-target damage and execution. Modifiers scale the challenge by increasing monster density, damage, speed, or adding delirium-like effects that intensify the fight. These modifiers create a direct link between risk and reward: harder waves yield more valuable loot but demand tighter play.

Sustained defense: End-game runs hinge on maintaining a robust defense—solid health or energy shield, capped resistances, appropriate mitigation, and reliable flask uptime. A strong defensive baseline reduces the probability of a one-shot and keeps you in the fight longer.
Clean, scalable damage: A steady damage cadence that remains effective through exploding delirium and adds is crucial. You’ll want a rotation that can ramp up during safe windows and remain usable during chaotic moments.
Efficient adds management: Wave density means adds can overwhelm quickly. Efficiently thinning crowds while keeping the boss targets focused is key to progressing through waves without burning through your flasks or HP too quickly.
Loot optimization: Peaks in later waves reward better loot and currency. Weigh the potential payoff against the risk and time cost; if a wave pushes you past your comfort zone, it may be wiser to reset and refine your approach.
Entrance tips and readiness
Practice the mechanics in safer modes or lower-difficulty runs to internalize wave timings, delirium spikes, and boss patterns.
Ensure your resistances, HP, and mitigation meet the demands of late waves. A well-rounded toolkit—defensive auras, flasks, movement abilities, and crowd-control options—will pay dividends.
Have a plan for adds and arena hazards. Positioning and terrain use can dramatically reduce exposure to heavy damage zones and keep your DPS steady.
